Kamble R R, Shinde V S, Madhale S P, Kamble A A, Ravikumar B P, Jadhav R SIndian Journal of Medical Microbiology 2010 28(1):57-59Abstract Identification of Mycobacterium leprae, which causes leprosy, is done by Ziehl Neelsen Carbol Fuchsin (ZNCF) stained slit skin smear microscopy that ---- in the diagnosis and quantification of approximate bacterial load carried by the patient. We attempted M. leprae DNA extraction from 46 stained slit skin smear negative slides, using Proteinase K and SDS lysis, followed by ethanol precipitation. M. leprae specific primers (16SrRNA) were used for PCR-based amplification of DNA. We could detect M. leprae DNA in 15 (32.6%) samples. The method can be useful in the diagnosis of apparently slit skin smear negative leprosy cases. (Source: Indian Journa...

An Official ATS Clinical Policy Statement: Congenital Central Hypoventilation Syndrome: Genetic Basis, Diagnosis, and Management.

Conclusions: A PHOX2B mutation is required to confirm the diagnosis of CCHS. Knowledge of the specific PHOX2B mutation ---- in anticipating the CCHS phenotype severity. Parents of patients with CCHS should be tested for PHOX2B mutations. Maintaining a high index of suspicion in cases of unexplained alveolar hypoventilation will likely identify a higher incidence of milder cases of CCHS. Recommended management options aimed toward maximizing safety and optimizing neurocognitive outcome include: (1) biannual then annual in-hospital comprehensive evaluation with (i) physiologic studies during awake and asleep states to assess ventilatory needs during varying levels of activity and concentration, in all stages of sleep, with spontaneous breathing, and with artificial ventilation, and to assess...

Primary central nervous system lymphoma: Prognostication as per international extranodal lymphoma study group score and reactive CD3 collar
Tue, 09 Mar 2010 16:15:37 +0100 | Journal of Postgraduate Medicine
Conclusion : Approximately 90% (27 cases) were associated with poor prognostic factors in the present study according to the IELSG score. Perivascular reactive CD3 collar was seen in cases with low to intermediate IELSG score. A larger study is required to further validate that the presence of reactive perivascular CD3 collar is associated with good prognosis. This histological marker could be supplemented with IELSG score to stratify the patients of primary CNS lymphoma according to their aggressiveness. (Source: Journal of Postgraduate Medicine)

Increase in CD4 cell counts between 2 and 3.5 years after initiation of antiretroviral therapy and determinants of CD4 progression in India
Tue, 09 Mar 2010 16:15:37 +0100 | Journal of Postgraduate Medicine
Conclusions : CD4 progression continues two years after ART in patients who had base level > 100 cells. Early initiation of ART is necessary before CD4 crashing to < 100 cells for increasing the survival function. (Source: Journal of Postgraduate Medicine)

Circumcision: Pros and cons
Tue, 09 Mar 2010 16:12:01 +0100 | Indian Journal of Urology
Berk Burgu, Ozgu Aydogdu, Semih Tangal, Tarkan SoygurIndian Journal of Urology 2010 26(1):12-15Circumcision is possibly the most frequently performed elective surgical procedure in men. It can simply be described as the excision of the preputium. There have been several studies about the association between circumcision and urinary tract infections (UTI). Many studies have demonstrated that the frequency of UTI increase in uncircumcised males, especially in the first year of life. This review discusses the embryology of the preputium, epidemiology, indications, complications and benefits of circumcision, as well as operation and anesthesiology techniques. It especially examines the association between UTI and circumcision and the importance of circumcision in congenital urinary system anom...

Bone marrow can harbor HIV-infected cells
Tue, 09 Mar 2010 16:00:00 +0100 | ScienceDaily Headlines
Antiviral drugs have reduced ---- to more of chronic disease rather than a death sentence, but why is the disease so hard to cure? New research shows that bone marrow, previously thought to be resistant to the virus, can contain latent forms of the infection. Targeting these reservoirs of latent cells may open the door to new treatments. (Source: ScienceDaily Headlines)
