Effect of aberrant vasculature during cryosurgery for prostate cancer. Tue, 30 Dec 2008 05:00:00 +0100 | Urology Effect of Aberrant Vasculature During Cryosurgery for Prostate Cancer. Urology. 2008 Dec 30; Authors: Jones JS Efficacy of cryosurgery for treatment of prostate cancer is based on completely freezing the prostate and all malignant tissues. The image from this patient demonstrates aberrant periprostatic vascular anatomy featuring two vessels that created a "heat sink" that overcame the outer extension of the ice ball, resulting in potential incomplete freeze in the area of the vessels. Wed, 10 Dec 2008 05:00:00 +0100 | Cryobiology This study is a part of an ongoing effort to develop computerized tools for cryosurgery. Based on typical clinical practices, previous automated planning schemes have required that all cryoprobes be aligned at a single insertion depth. The current study investigates the benefit of removing this constraint, in comparison with results based on uniform insertion depth planning as well as the so-called "pullback procedure". Planning is based on the so-called "bubble-packing method", and its quality is evaluated with bioheat transfer simulations. This study is based on five 3D prostate models, reconstructed from ultrasound imaging, and cryoprobe active length in the range of 15-35mm. The variable insertion depth technique is found to consistently provide superior results when compared to the ot... Prostate cryotherapy monitoring using vibroacoustography: preliminary results of an ex vivo study and technical feasibility Tue, 04 Nov 2008 05:00:00 +0100 | IEEE Transactions on Biomedical Engineering In conclusion, this pilot study shows that VA produces clear images of a frozen prostate at different temperature stages. The frozen tissue appears as a uniform region with well-defined borders that are readily identified. These characteristic images should allow safer and more efficient application of prostatic cryosurgery. These results provide substantial motivation to further investigate VA as a potential modality to monitor prostate cryotherapy intraoperatively. In the AUA 2007 Guideline for the Management of Clinically Localized Prostate Cancer: 2007 Update, multiple treatment modalities are considered as options. This conundrum is further complicated by stage migration and lead time bias, both associated with PSA-based early detection strategies and the resultant increase in the detection of small volume clinically localized cancers. Since the majority of men currently diagnosed with prostate cancer are likely to have the disease eradicated by one of several treatment modalities, the clinical focus on HRQL associated with treatment has intensified. There are no published l... They act as potent inducers of Th1 responses, including the activation of cytotoxic CD8+ T lymphocytes (CTL). Whereas animal models have provided clear evidence that CpG enhances antitumor immunity, clinical trials in humans have thus far been less successful. Applying cryosurgery as an instant in situ tumor destruction technique, we now show that timing of CpG administration crucially affects colocalization of antigen and CpG within EEA-1+ and LAMP-1+ compartments within dendritic cells in vivo. Moreover, antigen/CpG colocalization is directly correlated with antigen cross-presentation, the presence of CTL, and protective antitumor immunity. Thus, failure or success of CpG as a vaccine adjuv... Clinical study of cryosurgery efficacy in the treatment of skin and subcutaneous tumors in dogs and cats. Sat, 29 Mar 2008 04:00:00 +0100 | Medical Engineering and Physics In this study, we use multimodal multiphoton microscopy to assess the change of extracellular matrix after cryotreatment of skin. The cellular matrix transformations and the intercellular interactions during the wound healing process after cryolesion for mice were measured in vivo and in real-time through the dorsal skinfold chamber (DSC). Intrinsic second-harmonic generation (SHG) signals from fibrillar collagen and two-photon excited (TPE) autofluorescence from cell were collected to investigate the cryosurgical response in vivo. The TPE and SHG signals are significantly different between normal and cryotreated mice, and correlates with the wound healing process. It is a life-threatening prognostic aspect. Hepatic resection, when possible, is the best therapeutic modality, although the overall survival rate is still low (30%). Angiography and intraoperative ultrasonography are useful for resection. The number of hepatic metastases and the surgical margin are probably the most significant prognostic factors. Colorectal cancer may spread predominantly to the liver making regional treatment strategies viable options. Subtotal hepatic resections and segmentectomies are potentially curable procedures for single or small numbers of hepatic metastases without other sites of disease. However, there have...
